Cole's archetypal patient intuitively knows something is off — fatigue, weight gain, a digestive problem, an inflammatory problem, a mental health issue. They go to the doctor, get the basic labs, and the results come back quote-unquote normal. Nothing changes. Under the optimal-range method, the same blood is read against a tighter interval and a wider panel: glucose at 94 and triglycerides at 130 are technically in range but functionally flagged, and hs-CRP that was never ordered turns out to be the actual signal.
