Shah learned this from Bill Gates, who would pull the room together and ask something apparently naive: how much does it cost to vaccinate every child on the planet? The world's experts pushed back that it was too complex and too reductive to answer. That pushback is the signal. The question's power is not the number it produces but the forced analysis, forced learning and forced identification of barriers that answering it demands. Chasing it made the team model 105 million annual births, discover half went unvaccinated, quantify 11 million child deaths, and study vaccine supply and demand. That chain led past the obvious barriers — roads, power, cold chain — to the actual constraint: manufacturers had reformulated and shrunk the global supply base, so there simply weren't enough vaccines. The solution then designed itself against that one barrier.

Shah learned the method from Bill Gates at the Gates Foundation, where Gates's habit of asking deliberately simple questions repeatedly broke expert deadlock on global health problems.

How to run it

  1. 1

    Ask the naive, reductive question

    State the goal as one question a smart outsider would ask, ideally one that demands a number. 'How much does it cost to vaccinate every child on the planet?'

    Pro tip If experts in the room say the question is too simplistic to answer, you've found the right one.

    Refuse the complexity defence

    Don't accept 'it's hard to be that reductive'. Insist the question gets modelled anyway, with assumptions stated where data is missing.

    Watch out Deferring to expert discomfort here kills the whole method — this is the step most people skip.

    Let the answer force the analysis

    Working the question surfaces the sub-questions: who is and isn't served, what the consequence is, what the supply and demand actually look like.

    Pro tip Treat each sub-question as a mini research project rather than a guess to be filled in.

    Chase past the plausible barriers

    Lists of blockers — roads, power, refrigeration, cold chain — are usually cover for one real constraint. Keep asking why until only one remains.

    Pro tip Ask the barrier owner directly: why aren't you producing enough? Sit with them rather than analysing from afar.

    Watch out A long barrier list is what makes people give up. It is almost never all binding at once.

    Absorb the domain

    Learn every nook and cranny of the industry, its gaps and its incentives. Shah is explicit that fresh solutions come out of years of deep study and interviewing.

    Pro tip Interview practitioners, not only experts — the Ebola answer came from local communities, not epidemiologists.

    Watch out The nonprofit reflex is to gloss over this phase because it feels like delay rather than progress.

    Build the solution for the one constraint you named, even if it requires an instrument nobody has used before.

Why aren't there enough vaccines?

The complexity story said vaccinating every child required roads, power infrastructure, refrigeration and cold chain. Chasing the simple question found something narrower: manufacturers had changed formulations and dramatically shrunk the global supply base, so vaccines only existed for wealthier countries. Manufacturers would expand only with long-term contracts and confident demand. That constraint was answerable — the team created the world's first social impact bond with the UK, France and other European countries, raised $6 billion via debt issuance, and restructured how the world procured vaccines.

A vaccine supply base roughly three times larger, which became the solution the whole big bet rested on.

Why are people contracting Ebola?

In the 2014 West Africa Ebola response, the simple question was why transmission kept happening. The answer came not from experts in Atlanta, New York or Geneva but from local communities: traditional Liberian burial practices involved washing, hugging and redressing the bodies of deceased family members. The response designed a process around that one fact — WHO-approved body bags, teams in protective equipment, a ceremony to honour the deceased, then removal before further contagion.

A 70% reduction in case transmission; the CDC had projected 1.4 million cases and the outbreak ended at roughly 30,000.

Common mistakes

Letting expertise add complexity instead of removing it

As careers deepen, people overwhelm their own thinking with complexity and lose the ability to ask the reductive question.

Asking the question but not chasing it

The question is worthless without the years of forced analysis behind it. Shah's own closing advice is to ask a simple question and 'really chase it down'.

Looking for the answer only among credentialed experts

The Ebola breakthrough came from local people who understood their own cultural practices, not from institutional epidemiologists.
